Job Summary:
The CMT, under the direction of the nurse, will administer medications to patients. They function under the direction, instruction and supervision of the staff nurse and the Nursing Director and/or appropriate supervisor.
Organizational Relationship: The CMT reports directly to the nurse manager
Risk of Occupational Exposure To Blood Borne Pathogens: A: High Exposure
Education:
- High School graduate or equivalent.
- Graduate of an accredited Certified Medication Technician Course.
- Currently certified by the Board as a Certified Medication Technician.
Experience:
One (1) year related experience in a medical environment preferred.
Licenses/Certifications:
- Must be certified as a Medication Technician in the state within which services are provided.
- Must have current CPR Certification.
- Preparing and administering prescribed medications to patients under the supervision of a
licensed RN, and in compliance with all Agency policies and procedures and Pharmacy Board
rules and regulations governing the administration and documentation of medication. - Adhering to all Agency policies and procedures and Pharmacy Board rules and regulation
regarding the administration and documentation of narcotic medications. - Performing medication administration responsibilities while adhering to all safety principles.
- Documenting administration of medication on the Medication Administration Record (MAR). •
Documenting medications not administered on the MAR, and the reason for not being
administered. - Adhering to proper infection control techniques during the medication administration process,
including the incorporation of universal precautions when indicated. - Recognizing and adhering to the six (6) rights of medication administration.
- Participating in data collection, medication audits, and other performance improvement activities related to medication administration.
- Reporting any adverse patient medication reactions immediately to a supervisor.
- Reordering and replacing medications and medication administration supplies, as needed, to perform routine job responsibilities.
- Ensuring the confidentiality and rights of patients, the confidentiality of the health systems and department documents, and adhering to HIPAA Regulations.
- Must be able to apply principles of critical thinking to a variety of practical and emergent situations and accurately follow standardized procedures that may call for deviations.
- Must always be alert; paying close attention to details.
- Be able to communicate effectively in English.
- Perform other duties as assigned.
Functional Abilities:

Job Summary:
The home health aide provides personal care and related services in the home under the direction, instruction and supervision of the staff RN and the Nursing Director and/or appropriate supervisor.
Organizational Relationship: The HHA reports directly to the RN Supervisor
Risk Of Occupational Exposure To Blood Borne Pathogens: A: High Exposure
Qualifications:
- Completion of a MD approved HHA course and competency and have received additional home care training (including communication skills, techniques of home client care, maintenance of clean, safe home environment, food & nutrition, infection control, caring for clients with special needs, caring for clients with terminal needs) and been skills competency tested.
- Experience as a HHA for at least 1 year within the past 3 years.
- Must have current CPR certification.
Responsibilities:
Tasks to be performed by a HHA must be assigned by and performed under the supervision of an RN who will be responsible for the client care provided by the HHA.
Duties of the HHA may include:
- The provision of personal care services/assist with ADLs and iADLs as per the POC.
- To follow the POC for each client and document the same.
- Helps the client to maintain good personal hygiene.
- Assists in maintaining a healthful, safe environment.
- Plans and prepares nutritious meals. Markets when indicated on the POC.
- Assists the client with ambulation as per the POC
- Assists with assigned treatments as ordered by the physician and approved/supervised by the nurse.
- May cue for medications ordinarily self-administered as assigned.
- May administer medications if certified as a certified medication technician.
- Taking and charting vital signs.
- Encourages the client to become as independent as possible according to the nursing care plan.
- Attempts to promote client’s mental alertness through involvement in activities.
- Gives simple emotional and psychological support to the client and other members of the household.
- Establishes a relationship with client and family which transmits trust and confidentiality.
- Prepares a report of her visit on the day it is performed and incorporates same in the clinical record weekly or as directed.
- Reports any change in the client’s status or home situation to her immediate supervisor, the staff nurse, or the aide supervisor.
- Carries out their assignment as instructed by the nurse or the paramedical team and documents and reports to the nurse when she is unable to do so.
- Works with personnel of other community agencies involved in the client’s care as directed by the staff nurse.
- Performs routine housekeeping tasks as related to a safe and comfortable environment for the client, as instructed by the professional nurse.
- Attends in-service as required by state and federal regulations.
- Confirming on a weekly basis, the scheduling of visits with the Supervisor/Director, to coordinate necessary visits with other personnel.
- Notifying the RSA of absences due to illness, emergency leave, normal vacation periods, or special professional meetings, which will affect agreed service with the RSA.
- Wears the RSA’s name badge at all time they are seeing RSA clients.
Functional Abilities:
- Must be able to read 12 point or larger type.
- Must be able to hear and effectively communicate in English.

Job Summary:
A nurse currently licensed and registered who provides professional services within the scope of
nursing practice standards in collaboration with primary care physicians, teaches and educates
clients and their families.
Organizational Relationship: Reports to the Nursing Director

Qualifications:
- Graduate of an approved professional nursing program (RN).
- Currently, licensed “in good standing” in MD.
- Minimum of one (1) year of experience as a professional nurse, preferably in home care.
- Must have a criminal background check.
- Must have current CPR certification.
Responsibilities:
- Receives the intake referral information and prepares paperwork/tools necessary for the visit.
- Performs assessments, including medical history from client, family or significant others, which includes client/family involvement.
- Performs home safety check and environmental assessment of the client’s home environment.
- Performs the socio-psychological evaluation of the support systems available to the client and documents necessary emergency contacts etc.
- Performs assessment visit and documents timely. (i.e. assessments/ Skilled Nursing Note etc.).
- Performs physical examination and review of all body systems and documents such accordingly.
- Develops an appropriate and effective Plan of Care (POC) to be submitted to the physician for approval and implementation.
- Determines necessity for other services that could enhance the positive outcome desirable for the case.
- Evaluated the client’s ADL and IADL abilities and therefore need of support services such as aides.
- Develops and implements the Aide plan of care when Aide services are ordered. Revises and signs the care plan.
- Supervises the Aides in accordance with state/federal requirements and documents the supervision without having to be directed to do so.
- Orders “other” professional services that are effective and is willing adjust frequency of these services as outcome progresses and the client’s condition warrants.
- Reviews billing processes with client and/or family advising client and/or family when co-pay or insurance is not likely to pay for services.
- Effectively communicates with client and family the POC and progression of such. Keeps the client informed ongoing.
- Effectively communicates with other disciplines in the case (case conferencing) to effectively and appropriately problem solve as situations arise.
- Communicates effectively with the Nursing Director/RN Supervisor scheduled visits planned and changes to the schedule on a weekly basis.
- Communication with the client’s physician (verbally and/or in writing) to obtain effective treatment modalities and/or rehabilitative therapy modalities to effect the best means to obtain the desired outcome for the specific medical problem that caused the case to open and/or recertify.
- Communicates in the case conferencing sessions to establish best practices for the individual client’s needs.
- Submits accurate, complete paperwork at the end of every week so that all medical records are intact and up to date. Uses the drop box if the office is already closed for the weekend.
- Submits ALL assessment documentation within 48 hours of the visit.
- Submits requests for more visits needed prior to third party insurers authorized number of visits expiring.
- Coordinates Community Services that may be available to the client to assist in safe home care needs.
- Understands that the fiscal and clinical management of each case is directly linked to the success of his/her office.
- Participates in the Performance Improvement Committee process as requested.
- Performs clinical record reviews (CRR) per RSA policy in collaboration with the RN Supervisor.
- Submits the CRR timely.
- Participates in the staff meetings as part of the requirements of the position. Adjusts visits on the staff meeting day to accommodate this important function.
- Participates in the growth of the RSA by being a willing preceptor for newly hired, same discipline employees.
- Participates in the planning, operation and evaluation of the nursing services of the organization.
- Maintains professional licensure and actively seeks out educational experiences to enhance the practice of home care nursing for her/him and the benefit of the clients serviced.
- Notifies the RSA of emergencies, sickness, and other imminent occurrences that may affect the client caseload as quickly as possible relative to the event’s occurrence.
- Submits written time requests 2 weeks or more in advance of planned time off.

Job Summary:
A Licensed Practical Nurse (LPN) provides practical nursing skills within the scope of nursing practice standards in collaboration with RN case managers/supervisors, teaches and educates clients and their families according to the established professional plan of care.
Organizational Relationship: Reports directly to the assigned RN

Qualifications:
- Graduate of an approved LPN nursing school.
- Currently, licensed “in good standing” in MD.
- A minimum of one (1) year of experience as a LPN, preferably in a home care setting.
- Must have a criminal background check.
- Must have current CPR certification.
Responsibilities:
- Maintains the nursing frequency established by the supervising RN.
- Follows the plan of care established by the supervising RN.
- Prepares equipment & materials for treatments observing aseptic techniques as required.
- Performs home safety check and environmental check of the client’s home environment on all visits. Reports any unsafe or potentially unsafe condition to the supervising RN.
- Performs nursing visit and documents accordingly (Skilled Nursing Note etc.).
- Reviews and follows the supervising RN’s plan of care, communicating frequently with the supervising RN on any suggested changes necessary.
- Collaborates with the supervising RN on a weekly (or more frequent basis) to benchmark toward the established outcome goals.
- Reviews the client’s ADL and IADL abilities and therefore need of support services such as home health aide. Communicates any suggested changes to the supervising RN.
- Reinforces the Aide plan of care when Aide services are ordered.
- Effectively communicates with client and family the POC and progression of such. Keeps the
client informed ongoing. - Effectively communicates with other disciplines in the case (actively participates in case conferencing) to effectively and appropriately problem solve as situations arise.
- Communicates effectively with the supervising RN, scheduled visits planned and changes to the schedule on a weekly basis. Caseload is self-scheduled but communication of the clinician’s schedule to the RN Supervisor is essential.
- Communicates in case conferencing sessions. Effectively functioning as an involved team
member to collaborate on best practices for the individual client’s needs. - Submits accurate, complete paperwork at the end of every week so that all medical records are intact and up to date. Uses the drop box if the office is already closed for the weekend.
- Notifies the supervising RN with requests for re-authorization of “more visits needed” prior to third party insurers authorized number of visits expiring.
- Suggests Community Services that may be available to the client to assist in safe home care needs.
- Understands that the fiscal and clinical management of each case is directly linked to the success of his/her office.
- Participates in the PI/QA Committee process as requested.
- Participates in the staff meeting as part of the requirements of the position. Adjusts visits on

Qualifications (Preferred unless required by law):
- Completion of the 9th grade; prefer high school graduate, GED is acceptable.
- Successful completed a MD/Board approved CNA course and competency and have received
additional home care training (including communication skills, techniques of home client care, maintenance of clean, safe home environment, food & nutrition, infection control, caring for clients with special needs, caring for clients with terminal needs) and been skills competency tested. - Experience as a CNA for at least 1 year within the past 3 years.
- Must have current CPR certification.
- If providing medication administration, is a MD Certified Medicine Aide.
Responsibilities:
- Take vital signs (temperature, blood pressure, pulse, and respiratory rate).
- Provide direct patient care such as assisting with bathing, eating, dressing, and walking patients.
- Turn and reposition bedridden patients to prevent breakdown of their skin.
- Change bed linens.
- Record amount of oral intake and measure urinary output.
- Transfer patient using Hoyer lift or assist patients in transferring from bed to chair, chair to chair or chair to bed as necessary.
- Assist patient in ambulation with/or without assistive devices such as walkers and canes.
- Collect non-invasive specimens for tests.
- Position and empty bedpans and/or urinals as necessary.
- Interact with patients and family.
- Transport patients (if allowable) and equipment as needed per Agency policy.
- Keep a record of care given.
- Monitor patients and report any variances to normal to the nurse for further assessment.
- Follow universal precautions to prevent the spread of organisms.
- Carries out her assignment as instructed by the nurse or the paramedical team and reports to the nurse when she is unable to do so.
- Works with personnel of other community agencies involved in the patient’s care as directed by the staff nurse
- Performs routine housekeeping tasks as related to a safe and comfortable environment for the patient, as instructed by the professional nurse
- Attends in service as required by state and federal regulations.

must occur 3 hours or more prior to visit scheduled. - Demonstrates patience, honesty, good health, and the willingness and ability to follow directions.
- If providing medication administration, is a certified Medicine aide providing only those services allowed under the certification.
